Before beginning to develop a new e-Form that will be placed in the central e-Forms library, one must first obtain a Form ID. Form ID's are assigned by the e-Forms administrator, and are used to catalog and link together all the support documentation associated with a specific electronic form. e-Form developers must include a Form ID in the footer area of their forms. For additional information, please also review e-Forms Guidelines and Standards. and Information for e-Form Authors. To request a Form ID, fill out and submit the form below. The e-Forms administrator will reply to your request via email, and provide an e-Form ID to use with your new form.
